How to Track Your Symptoms & Observations

Observations are where you can track symptoms, sleep, diet, or anything else related to your health. Every check-in adds up — giving you more personalized, actionable insights over time.

This guide will walk through:


Everyone will start with two default observations, "Meals & Diet" and "Sleep." You'll also see any other symptoms or observations you selected when you set up your account.

How do I track a symptom or observation?

To track, simply tap on the card, answer the question, and tap "Done" or the green button on the bottom right to save your answer.

To start over, tap "Clear" in the top right corner.

Navigating through observations & adding context

You can navigate to prior or following observations with the arrows on the bottom of the screen.

You can add more context about the symptom or observation if you expand "Additional Details", including tags (link to Tag Tracking), time, duration, notes, and images.

Can I track the same symptom more than once in a day?

Yes! You can add multiple instances of the same observation as you track throughout the day. Tap on the + sign on the upper left corner to add a new instance of your symptom.


How do I add a new symptom or observation?

To add a new observation, click on the "+Add" button in the top right of the Observations section, or the "+Add an observation" button at the bottom of the section. You'll see a list of anything you're actively tracking, have previously tracked, or you can search for something completely new!


What is a baseline and how do I set it?

Your baseline is what's normal for you. For example, if you usually have a mild headache, you'd set your Headache baseline to Mild.

Baselines are most commonly used for symptoms and observations you track on a scale of 0-5, also called a severity scale.

Why should I set a baseline?

Once you've set a baseline, you can filter your calendar to see when a symptom was above or below your normal. Your baseline also helps Folia give you more accurate flare suggestions when you track something more severe than usual.

How do I set a baseline?

We'll let you know any of your symptoms or observations need a baseline with the purple card at the top of your Observations section. Simply fill out your "normal" and set your baseline. *Please note setting baselines will not track the symptom/observation for that day.


How can I customize symptoms or observations?

In Folia, you can track symptoms and observations in several ways, and you're free to change how you track them at any time. The recommendations below are just a starting point for how you can think about using each type of observation— your health is personal, and your journal should be too.

  • Severity scale (0-5 rating system): Best for symptoms you experience daily or regularly

  • Yes/No: Best for symptoms you don't experience as often

  • Count: Best for tracking how many times something happens in a day — like glasses of water consumed

  • Tags: Best for observations where you want to capture notes in a structured way

All observation customizations will start in the same place:

  1. Tap the gear icon in the top right of the Observations section

  2. Tap "Edit" on the observation card you want to edit

From here, to change how you track an observation:

  1. Tap "Edit" in the Observation Type section

  2. Select the new type of observation you'd like to track with and "Save"

To add or edit a baseline:

Baseline recommendations will only appear for severity observations (0-5 scale). However, you can add baselines or edit them from Observations settings.

  1. Tap "Edit" in the Baseline section

  2. Select your new baseline and "Save"

To remove, add, or change a tracking schedule:

When you add a new observation, it will default to appear in your Journal daily. However, you can remove symptoms/observations that no longer feel relevant or change the tracking schedule at any time.

  • To remove the tracking schedule: Tap the red trashcan to delete the schedule

  • To add a schedule: Tap "+Add schedule"

  • To edit the Tap "Edit" in the Tracking Schedule section and tap on the existing schedule to change/customize it
