What do I earn gift cards for?
There are two main study activities for which you'll receive gift cards.
Journal Tracking: In the Journal tab (the calendar icon on the bottom toolbar in your app) you'll see Observations and Treatments. You can add any symptoms you're experiencing or treatments you're taking. Here's a step-by-step guide to tracking in your Journal. At least once a week, go into your Journal and track these symptoms and treatments for the day (ex. if you're tracking fatigue, select Fatigue in observations and use the 0 to 5 scale to tell us about your fatigue today).
Monthly Reflection Surveys: On the first of each month, your reflection survey will be available in the Research Checklist through the 15th of the month. Complete Part 1 and Part 2 of this survey in the Folia app.
Who is eligible to earn gift cards?
To be eligible for gift cards, participants must upload a confirmation of your CIDP diagnosis (i.e doctor's note, health portal screenshot, etc.).
When will I receive gift cards?
Gift cards are sent out monthly on the 28th to participants that have uploaded their confirmation of CIDP diagnosis. For example, gift cards for May tracking and the May monthly survey will be sent out on June 28th.
How do I redeem my rewards?
Here's how to redeem your e-gift cards in the Folia Health app.
1. Open the Folia app and click on the Profile tab
2. Click on the "$" button in the top-right
3. Redeem available gift-cards
What kinds of gift cards can I choose from?
We issue gift cards using Tremendous, so you'll be able to choose from a variety of US retailers and prepaid Visa cards.
Note: Gift cards are only redeemable in the US and US territories.
